SIGN UP RELEASE AND WAIVER AGREEMENT
TRAPPED ENTERTAINMENT LTD.
PARTICIPATION WAIVER AND RELEASE OF LIABILITY
1. Voluntary Participation and Assumption of Risk
Trapped Entertainment Ltd. ("Trapped") operates interactive escape games and related attractions. I acknowledge that participation is voluntary and may involve risks, including slips, trips, falls, collisions with objects or other participants, cuts, bruises, strains, anxiety, stress, flashing lights, dark spaces, confined areas, and injuries arising from interaction with props, locks, doors, furnishings, or other equipment.
2. Assumption of Risk
I freely accept and assume all risks of injury, illness, emotional distress, death, property damage, or other loss arising from or related to my participation, whether foreseen or unforeseen.
3. Release of Liability
In consideration of being allowed to participate, I release, waive, and discharge Trapped Entertainment Ltd., its owners, directors, officers, employees, contractors, agents, landlords, and property owners from any and all claims, demands, actions, damages, losses, costs, or expenses arising out of or related to my participation, including claims for personal injury, death, or property damage. This release includes, to the fullest extent permitted by law, claims arising from the negligence of Trapped Entertainment Ltd. or its staff, except where prohibited by law.
4. Health and Safety
I confirm that I am physically and mentally able to participate. I understand that some games may involve dark spaces, flashing lights, loud noises, sudden discoveries, or confined areas. I agree to notify Trapped staff immediately if I feel unwell, unsafe, distressed, or unable to continue. I understand that I may leave the game at any time if needed.
5. Participant Responsibilities
I agree to follow all posted rules and all verbal instructions given by Trapped staff. I must not participate while under the influence of alcohol or drugs, bring weapons or dangerous items onto the premises, use force on doors, walls, furniture, locks, props, or equipment, behave in a reckless, unsafe, abusive, or disruptive manner, or use recording devices in any game room unless expressly permitted by Trapped. Failure to follow the rules or staff instructions may result in removal from the activity without refund.
6. Damage, Media, and Refund Policy
I am responsible for intentional or reckless damage caused by me to Trapped property, equipment, fixtures, or game contents, and I may be required to pay the reasonable cost of repair or replacement. Unless I advise Trapped staff otherwise before the activity begins, I consent to Trapped using photos, video, or audio containing my likeness or voice, taken before, during, or after participation, for promotional, advertising, or social media purposes, without compensation. I understand that Trapped booking and refund policies apply to my reservation and participation, and that removal for rule violations or unsafe behaviour will not result in a refund.
Age and Supervision Policy: Participants aged 16 and older may sign their own waiver. Participants aged 15 and younger must have a parent or legal guardian sign on their behalf on the reverse side of this form. Participants aged 14 and younger must have a responsible adult either participating in the room with them or remaining in the building for the full duration of the game.
I confirm that I have read and understood this Participation Waiver and Release of Liability, and I sign it voluntarily.
