CHILD WAIVER AND PARENT / LEGAL GUARDIAN CONSENT
This form is for participants aged 15 and younger only. Participants aged 16 and older may sign their own waiver. A parent or legal guardian must complete this form for any participant aged 15 and younger before participation in activities at Trapped Entertainment Ltd. (“Trapped”).
Trapped Entertainment Ltd. operates interactive escape games and related attractions. Participation in an escape room is voluntary and may involve certain risks, including but not limited to slips, trips, falls, collisions with objects or other participants, cuts, bruises, strains, anxiety, stress, flashing lights, dark spaces, confined areas, and injuries arising from interaction with props, locks, doors, furnishings, or other equipment.
By completing this form, I confirm that I am the parent or legal guardian of the minor named below, that I understand the risks associated with participation, and that I give permission for the minor to participate in activities at Trapped.
In consideration of the minor being allowed to participate, I agree, on my own behalf and on behalf of the minor, to release and discharge Trapped Entertainment Ltd., its owners, directors, officers, employees, contractors, agents, landlords, and property owners from any and all claims, demands, actions, damages, losses, costs, or expenses arising out of or related to the minor’s participation, including claims for personal injury, death, or property damage, to the fullest extent permitted by law. This includes claims arising from the negligence of Trapped Entertainment Ltd. or its staff, except where prohibited by law.
I confirm that the minor is physically and mentally able to participate. I understand that some games may involve dark spaces, flashing lights, loud noises, sudden discoveries, or confined areas. I agree that the minor will follow all posted rules and all instructions given by Trapped staff. I understand that unsafe, reckless, abusive, or disruptive behaviour, or failure to follow staff instructions, may result in removal from the activity without refund.
I understand that participants aged 14 and younger must have a responsible adult either participating in the room with them or remaining in the building for the full duration of the game.
I understand that Trapped’s booking and refund policies apply to this reservation and participation. Unless I notify Trapped staff otherwise before the activity begins, I also consent to Trapped using photos, video, or audio containing the minor’s likeness or voice, taken before, during, or after participation, for promotional, advertising, or social media purposes, without compensation.
Emergency medical consent
In the event of an emergency, I authorize Trapped Entertainment Ltd. to seek medical assistance for the minor if I cannot be reached immediately. I understand that I am responsible for any costs associated with such care.
